Who is this for? Learners who need to replace or reissue a student ID card.
You can request a reissued student ID card if yours is lost, damaged, or contains incorrect information, or if your name has been officially updated. Requests are made through the ID Card Re-issuance Form, and payment must be verified before processing begins.
Make payment using the designated link provided in the ID Card Re-issuance Form.
Your official Miva student email address.
Your programme of study.
Your Student ID or Matriculation Number.
Your first and last name, exactly as you want it to appear on the card.
Your reason for the request, such as a lost or damaged card, name correction, replacement, or incorrect information.
Confirmation that you have paid the applicable fee.
Open the ID Card Re-issuance Form and complete all fields accurately.
Make the applicable payment and verify it within the form.
Submit the form.
Once your form and payment are verified, the team reviews your request, reprints your card, and contacts you about delivery or collection. Processing times vary with request volume and payment verification.
Note Ensure every detail is accurate. Incorrect details can lead to an incorrectly reprinted card. Payment verification is mandatory before processing begins.
Can I update my name on the new card?
Yes, provided the name change has already been officially updated in the university's records.
